Our Mission

The Aspen Primary Care Network provides our patients with timely and responsive access to high-quality, comprehensive care aimed at reducing the burden of illness and promoting health through our multidisciplinary teams.

About the PCN

The Aspen Primary Care Network (PCN) represents the partnership between family physicians in Athabasca, Westlock, Barrhead, Swan Hills, Kinuso and High Prairie, Alberta. 

The PCN Business Plan was developed in 2008–09, based on the needs and demographics of the geographic areas we serve, and we work to achieve the provincial primary-care objectives within our Network. The Aspen PCN became operational April 1, 2010.

Our priorities are to facilitate health promotion and disease prevention, and to enable successful self-management of chronic diseases and conditions. In order to address our priority initiatives, we collaborate with our partner providers in health care, Alberta Health Services, other health-care agencies, and the local communities. ​

In total, the PCN has 26 participating physicians serving approximately 26,000 residents within our region. We focus on the development of strong multidisciplinary teams that include:

  • Licensed Practical Nurses
  • Exercise Specialists
  • Behavioral Health Practitioner​
