Diabetes Education & Wellness

Aspen PCN is excited to announce a new partnership with Alberta Health Services and other community organizations, to enhance the way we are providing diabetes education. This initiative will empower those living with diabetes or supporting         someone  living with diabetes, to take control of their health.

                                                MEET THE TEAM!

 This initial session will connect individuals with potential members of their health care team and allow them to choose tailored education sessions to fit their needs.  The team will provide education and support, and includes Chronic Disease Management Nurses, a Certified Diabetes Educator, a Registered Dietitian, an Exercise Specialist, Pharmacists, and a Social Worker.
