Health Home - Westlock

Aspen PCN Health Home in Westlock is conveniently located in downtown Westlock. Our team on site includes Nurses, a Certified Respiratory Educator, an Exercise Specialist, and a Behavioral Health Practitioner to assist patients referred by their physician. 

INDIVIDUAL SERVICES

  • INDIVIDUAL COUNSELLING – REQUIRES PHYSICIAN REFERRAL
    • Health counselling to facilitate successful self-management of chronic diseases such as diabetes, dyslipidemia, heart disease, COPD, etc.   
    • Behavioural counselling to facilitate successful self-management of issues related to stress, anxiety and depression.

  • INDIVIDUAL DIAGNOSTIC SERVICES – REQUIRES PHYSICIAN REFERRAL
    • Diabetic Foot Assessment 
    • Diabetic Foot Clinic 
    • Diabetic Retinopathy Eye Exams 
    • 24 HR. Blood Pressure Monitoring and Reporting 
    • Level 3 Sleep Studies 
 

VISITING SPECIALIST CLINICS – REQUIRES PHYSICIAN REFERRAL

The PCN accommodates various specialists within the Health Home in both Athabasca and Westlock in order that our patients do not have to travel to a large municipal centre to see a specialist – instead we bring the specialists to our PCN!  In Westlock these include:

  • Visiting Pulmonology Specialist – Healthy Lung Clinic, Breathe Easy Program
  • Visiting Surgical Specialist – referrals for general minor surgery and for diagnostic testing  such as colonoscopy, gastroscopy;  any resulting surgical treatments are performed in Barrhead 
  • Visiting Opthalmology Specialist – pre and post appointments for cataract surgeries done at Westlock Health Care Centre
  • Visiting Cardiology and Endocrinology (diabetes) Specialist
  • Visiting Hearing Practitioner
 

GROUP PROGRAMS

  • BREAK FREE – FREE 3 week group session designed to provide participant with information about nicotine addiction and techniques for successful cessation of tobacco use.

  • BREATHE EASY – FREE 8 week program provided in conjunction with Alberta Health Services to enable participants with chronic lung disease to better manage their disease and enjoy more quality of life.

  • COMPASS FOR THE CAREGIVER – FREE 9 week Caregivers Alberta group program provided in conjunction with Westlock FCSS to enable unpaid caregivers to care for themselves so they can better care for their care recipients.

  • DIABETES EDUCATION AND WELLNESS PROGRAM – FREE program operating in conjunction with Alberta Health Services, for all diagnosed diabetic patients.   The program includes a meet your team initial group session.   Participants are then encouraged to attend any of several education group sessions based on their interests, and additionally attend quarterly one-on-one appointments with a PCN Nurse. 

  • FITNESS FOR LIFE - $5.00 drop in fee - Conducted at the local Westlock Rotary Spirit Centre - An exercise class designed to encourage participants to safely increase their physical activity levels to improve strength, balance and aerobic endurance.​​


  • HAPPINESS BASICS – COST $20.00 for participant manual – 4 week or 7 week group sessions based on the science and research of what happy people do, how they think and purposefully create happiness for themselves.
