About the PCN
The Aspen Primary Care Network (PCN) first became operational in 2010, serving three communities. Since then, we have grown to include partnership between family physicians in Athabasca, Westlock, Barrhead, Swan Hills, Kinuso, High Prairie and Wabasca, Alberta. In total, the PCN has 32 participating physicians serving approximately 30,000 residents within our region.
Our priorities are to facilitate health promotion and disease prevention, and to enable successful self-management of chronic diseases and conditions. In order to address our priority initiatives, we have established multidisciplinary teams which include:
- Exercise Specialists
- Behaviour Health Practitioners
- Health Promotion Facilitators
- Clinical Support and Administrative Staff
These teams provide services in two ways: first with nursing support directly within several family practice clinics; secondly through focused individual health counselling services, group programs and information sessions, and visiting specialist clinics based at our two centrally located Health Homes in Athabasca and Westlock. Our physicians and staff members collaborate and partner with Alberta Health Services, other health service providers, local human service agencies and community groups in order to best coordinate and provide appropriate patient centered care and service for our patients.
