About The Aspen PCN

The Aspen Primary Care Network (PCN) first became operational in 2010, serving three communities. Since then, we have grown to include partnership between family physicians in Athabasca, Westlock, Barrhead, Swan Hills, Kinuso, High Prairie and Wabasca, Alberta. In total, the PCN has 43 participating physicians serving approximately 30,000 residents within our region.

Our priorities are to facilitate health promotion and disease prevention, and to enable successful self-management of chronic diseases and conditions.

Position Summary

The Aspen Primary Care Network (APCN) is currently seeking Nurse Practitioners (NP) to provide primary care services to residents of Wabasca, Calling Lake and the surrounding communities. The primary focus of the NP will be to provide care and management of patients living with chronic disease and multiple co-morbidities. Some travel may be required. Experience working with Indigenous communities and knowledge of Indigenous culture is an asset.

Primary Responsibilities
  • Promotes an understanding of the Primary Care Network service and benefits, for all stakeholders as part of the advancement of enhanced primary care
  • Participate in the care of individual patients, practice populations and associated communities
  • To assist in the development and delivery of chronic disease management and health promotion programs and provide appropriate care and treatment according to currently accepted evidence based practice.
  • To provide restricted activities and care within the full scope of practice as defined under the Health Professions Act (HPA)
  • To make professional, competent and autonomous decisions with patients as a member of the multidisciplinary team.
  • To act as a nursing resource to other multidisciplinary team members, healthcare institutions and community agencies or programs
  • Participate in evaluation and research as it pertains to the outreach and chronic disease management/ complex care programs and services
Qualifications
  • Master of Nursing, Family Nurse Practitioner focus
  • Previous clinical experience in a primary care setting preferred
  • Experience with delivering adult learning modules and course facilitation preferred
  • Experience working with an EMR system preferred
  • Demonstrated understanding of the Alberta privacy laws
Conditions of Employment
  • Security; proof of a clean criminal record is required.
  • Completion of reference checks
  • Current CARNA Registration (submitted yearly)(submitted yearly)
  • Copy of current CPR (submitted yearly)
  • Must have own vehicle and valid driver’s license
  • Covid 19 vaccinations
