About The Aspen PCN

The Aspen Primary Care Network (PCN) first became operational in 2010, serving three communities. Since then, we have grown to include partnership between family physicians in Athabasca, Westlock, Barrhead, Swan Hills, Kinuso, High Prairie and Wabasca, Alberta. In total, the PCN has 43 participating physicians serving approximately 30,000 residents within our region.

Our priorities are to facilitate health promotion and disease prevention, and to enable successful self-management of chronic diseases and conditions.

Position Summary

The Practice Facilitator plays a key role in implementing the Primary Care Transformation initiative by assisting clinic teams with improvement toward the pillars of the patient-centered medical home model.

What You Will Do
  • Partnering with PCN team members, physicians, and clinic staff to develop and implement work processes related to patient access, patient attachment, panel management, patient screening
  • Facilitate awareness and understanding of the concept of the patient-centered medical home related to team-based care, panel management, preventative care, and enhanced patient access
  • Assist teams to identify roles and develop work processes that improve efficiency and patient centered care
  • Support ongoing team meetings, reinforcing improvement concepts, objectives, and methods
  • Work with the PCN, physicians and clinic staff to leverage EMR software and other documentation systems to support patient care
  • Resource to clinics respecting EMR optimization
  • Support clinic teams to gather data and report on process and outcome measures
  • Liaise with PCN support team members within the AMA to ensure knowledge of best practices and to attain support as required
  • Mentor and coach clinic staff to implement projects and or new processes
  • Assist with the identification and implementation of improvement projects and provide project management support
  • Partner with interdisciplinary teams to determine the best methods of optimizing the EMR functionality
  • Training will be provided
What You Need To Succeed
  • High school diploma or GED
  • Experience working in primary care or community health is preferred
  • Equivalent combination of relevant education experience and expertise may be considered
  • Change management improvement methods and quality improvement principles and their application to primary health care
  • Previous experience as coach/mentor and or facilitator and experience working with family physicians, primary care teams and or other primary care practices
  • Strong oral, written and analytical skills; self directed and accountable and high level of perceptiveness
  • Ability to collaborate with other team members
  • Good working relationship with healthcare professionals, clients, and community members
  • Respect for privacy and confidentiality
  • Support change management and transformation initiatives
  • Facilitation, problem solving, decision making and conflict management skills.
  • Organizational and time management skills
  • Proficiency in use of electronic medical records programs
  • Training will be provided
