Aspen PCN Health Home in Athabasca is conveniently located right across the street from the Family Health Centre. Our team on site includes Nurses, a Certified Diabetes Educator, Certified Foot Care Nurses, and an Exercise Specialist.

Individual Services
Individual Counselling - Requires Physician Referral
  • Health counselling to facilitate successful self-management of chronic diseases such as diabetes, dyslipidemia, heart disease, COPD, etc.   
  • Behavioural counselling to facilitate successful self-management of issues related to stress, anxiety and depression.
Individual Diagnostic Services - Requires Physician Referral
  • Diabetic Foot Assessment 
  • Diabetic Foot Clinic 
  • Diabetic Retinopathy Eye Exams (by referral to Westlock Health Home) 
  • 24 HR. Blood Pressure Monitoring and Reporting 
  • Level 3 Sleep Studies (by referral to Westlock Health Home) 
Visiting Specialist Clinics - Requires Physician Referral
  • The PCN accommodates various specialists within the Health Home in both Athabasca and Westlock in order that our patients do not have to travel to a large municipal centre to see a specialist – instead we bring the specialists to our PCN!  In Athabasca  these include:

Fitness For Life
  • $5.00 drop in fee
  • Conducted at the local Athabasca Regional Multiplex
  • An exercise class designed to encourage participants to safely increase their physical activity levels to improve strength, balance and aerobic endurance.

  • Enables participants to increase physical activity levels, and improve strength and balance
  • Conducted on site at Pleasant Valley Lodge, Athabasca  Mondays & Thursdays 9:15 – 10am (For Lodge Residents)

Goals of Care & Personal Directives

FREE one on one information sessions provided by staff throughout the year at the Health Home location, that provide information about advance care planning generally and goals of care and personal directives specifically.

Health Basics
  • Cost: $20.00 for participant manual
  • 8-week group session where participants learn and practice making small steps that lead to big changes.
  • Participants learn to choose healthy eating, become more active, and find balance in life.
