Chronic Disease Management

​Your Chronic Disease Management (CDM) team works together with your family physician to help you manage your disease. Your team includes Registered Nurses, Licensed​ Practical Nurses, Behavioral Health Practitioners, Exercise Specialists, Certified Diabetes Educators and Certified Respiratory Educators.

Your team will provide you with an individual care plan and education specific to your needs and​ provide on-going support to promote self management of chronic illness, such as diabetes, cholesterol, high blood pressure, and weight.  The Behavioural Health Practitioner can provide behavioural health coping skills for stress, anxiety, loss and grief. Our Exercise Specialist can develop individualized exercises for those living with pain and provide group exercise programs.

Working together, your CDM team will make sure you get the right care from the right professional at the right time.
